OACP's Trusted Assessor Service provides support to care providers Monday - Friday at the John Radcliffe Hospital to expedite discharge.

We take referrals on:

• local authority funded care home placements
• self-funders seeking a care home placement
• self-funders seeking a home care package
• complex local authority funded home care packages

• 581 assessments completed to 31 October saving the local system £1,472,835
• Plus 1,371 daily updates as requested by care homes

Saving care homes…

• 2,324 hrs at 4 hrs per assessment
• 3,486 hrs at 6 hrs per assessment
• At a total cost to the Oxfordshire system of just £42,000 to 30/10/2024
